Any medication given at school must come in its original container and must be accompanied by the Parent/Guardian Medication Authorization Form. (This includes over-the-counter medication and cough drops.)
Any prescribed medication must also be accompanied by the Medical Provider Authorization Form. Medication will NOT be given to the students if these proper forms are not accompanied with the medicine.
For those using Asthma Inhalers – Please fill out the Asthma Inhaler Administration Authorization Form.
All medication should be given to the Principal or the classroom teacher immediately upon entering the school building. Any medication that is not handed over will be confiscated and given to the parents. (See the Disposal of Medication Policy below.)
